PCN Care Co-ordinator (Morley & District PCN, Leeds)
Exciting role with Morley and District PCN (6 practices, 65k patients) to coordinate care for frail/elderly and long-term condition patients. Work with GPs/multi-disciplinary teams to create personalised care plans, navigate services, manage caseloads, and improve health outcomes. Community-facing role shaping local primary care.
Closes 30 Jan 2026.
Key responsibilities:
Develop/review personalised care & support plans
Coordinate appointments across health/social care services
Support patients/families with condition management & transitions
Liaise with PCN teams, record using SNOMED codes
Requirements: GCSE English/Maths, care coordination or health/social care experience, System 1 knowledge, own transport. Admin/people skills a plus.
